Adults: $325 Yearly (regularly $410-$450)

• Includes two cleanings (prophylaxis), two exams, four bitewing X-rays or one full mouth X-ray as needed
• 20% off any other treatment
• Additional family members (over 15yrs) $300 yearly

Children: $265 Yearly (Under 15yrs) (regularly $345-$380)

• Includes two cleanings (prophylaxis), two exams, four bitewing X-rays or one full mouth X-ray as needed, and fluoride treatment once a year
• 20% off any other treatment
• Additional children (under 15yrs) $225 yearly

Periodontal Maintenance Patients: $365 yearly (regularly $465-$515)

• Includes two periodontal maintenance, two exams, four bitewing X-rays or one full mouth X-ray as needed
• 20% off any other treatment

Details and Restrictions

• Membership fee is due upon joining and membership is renewed every 12 months
• Cannot be combined with any other discount or insurance plan
• Services must be performed within the membership period
• Non-refundable or transferable
• Items/procedures not included in the 20% off:
  • Whitening procedures or refills
  • Philips Sonicare toothbrushes
